Yes.

Different Dodge values stack, +2 Dodge Chaosgarde and +3 Dodge Docent stack.
+2 Chaosgarde and +3 Dodge Chattering ring stack.
+3 Dodge Docent and +3 Dodge Chattering ring (same Dodge value) don't stack.

Yes, they do not stack.
I can't say for certain (only a dev could), but I suspect this is an early example of the ole "Turbine Special". See, they didn't want two chattering rings to stack, because it would have been way too overpowered at the time. The easiest way they could find to make that happen was to make equal values of dodge not stack. This worked pretty well for the chattering ring, and there was even a nice little bit of in-game flavor to go with it: the description on the ring says that wearing two causes them to try and talk over one another (hehehe). Unfortunately, when they came out with DT armor, this rule applied.
I suspect they are happy with it the way it is and won't change it.

Reportedly, some +1 Dodge bonuses can sometimes stack with each other in DDO as an *exception* to the rule that Dodge bonuses of equal value don't stack. Sometimes, but not always.
Least that's what I've read somewhere, though I can't provide any actual details nor examples ... sorry :(

I think that's probably possible via different types of sources: item, feat, spell.
So for example, the +4 dodge from icy raiment will stack with the +4 dodge from Inspire Heroics.
