You generate Ki for each melee attack you do, concentration is used to see how much Ki you can 'store'. Basicly punch (using a non Ki attack) get Ki, and profit :)

Note that you only generate ki if you do more than 0 damage i.e. bypass the DR on the mob.
Fire stance gives extra +1 to ki per hit
Oremi's necklace (tower of despair, high end raid) gives extra +1 to ki per hit
Way of the Crane gives extra ki on criticals depending on how many levels you take.

Isn't there something that gives passive Ki generation too?

Yes, at third or second level you get meditation 2x per rest to generate ki.

My Monk is only lvl 12 right now however I did not notice any passive KI Generation. You will be given a certain amount when you enter a dungeon (concentration total modifier). Other than that you gain ki from fighting and from meditating. Lvl 20 capstone is supposed to give you passive Ki Gen.

Non-Fire stance generate 1 ki/hit and 3ki/crit
Fire stances generate an extra 1ki/hit and some more to crits
path of crane give extra to ki/crit
and passive generation at lower levels is only avalable through the path of the badger, which generates ki when below 10-30%
