I'm going to post a Q&A here for the sake of those who are new to Green Steel as some of these things go unanswered or don't have an easy or quick-to-find answer.
I know the answer to most of these questions, but I want it to be helpful for other players as well.
If anyone else has a question, feel free to post it. I will generate a complete FAQ afterwords.
Please don't post an answer unless you're 100% sure.

1) Do all Green Steel effects stack with other effects on the SAME green steel item? (e.g. +50SP, +100SP, Wizardry VI.)
2) Do multiple Green Steel Items Effects stack with EACH OTHER?
3) Do Green Steel Item Effects stack with Normal Items effects?
4) How does wearing/making Multiple Green Steel Items (weapons and/or items) work and how does the Cleansing Orb come into play?
5) My Green Steel Items are causing me to lose hit points. What's the problem? (See FAQ #4)
6a) What do I get on my 20th COMPLETED Shroud run?
6b) Does this reoccur every 20th completion?
6c) Do I have to complete The Shroud 20 times on the same difficulty or does any difficulty count?
7) If I lose or use my Shavarath Stone, can I not enter the Shroud until I make a new one?
8) What effect does a Double-Imbued Shard of Supreme Power have on your Green Steel Item?
9) There are multiple websites with Green Steel preparation charts and programs. Which one is the most popular/correct/up-to-date one out there?
10) If I make a mistake, can a Green Steel Item be re-upgraded or downgraded?
11) Can I give a Green Steel Item to a friend, or make one for another of my toons who can't run the Shroud?
12) What level do I have to be to use a Green Steel weapon or item?
13) What do I do with Depleted Shavarath (Low, Medium, High) Energy Cells?
14) Can I make ANYTHING from Green Steel? Like Armor, Shields or Rings?
15) Can I share my ingredients with other toons or friends?
16) Can my Green Steel item be Attuned at the Alter of Change or upgraded via the Adamantine Ritual?
17) I have a Green Steel item that gives me permanent blur, but I don't see an icon or shimmering around my toon. Is it working?
18) What are the odds that my special Guard or Damage will take effect (e.g. Lightning Strike, Slay Living Guard) if (Green Steel Planner Website of Choice).com doesn't give me a straight answer?
19) Do Green Steel Special Effects (e.g. Lightning Strike, Slay Living Guard) work on Named Enemies? (e.g. Orange, Red or Purple Names).
20) What can I do with a Green Steel Item I don't want anymore?

1) yes, as it says so in their description
2) no, as it says so in their description
3) no, see 2
4) put 2 on and you wear 2. the cleaing essence is used in the part5 altar with your item and a high power cell
5) thats cause they have that effect on it. to remove it, you have to cleans one of them, see 4
6a) a choice of a cleaning essence or a +2 tome of your choice with the chance for +3 tomes
6b) yes
6c) just completion
7) yes, the stone has no impact if you can enter or not
8) that depends. mineral is differant then radiance and so on. and seeing that for every dualshard there are 6 possible effects, and like, dunno, 8 dualshard recipes out there, i wont list all those 48 differant here now

10) no, youre stuck with whatever you do
11) as long you havent equipped, upgraded or cleansed yet, you can give it around. otherwise its bound
12) 12
13) you charge them in the altars with weapons: medium cells in part3 with a +7 or higher weapon, high cells in part 5 with +8 or higher weapons (the number in the right top corner)
14) just weapons and items, no rings, no shields, no armors
15) yes
16) yes
17) yes
18) depends on the effect again. lightning strike is like 1-2%, slay living 5%. search this forums, theres a thread about that where all procs are lsited
19) if those arent immune, yes
20) sell it, but it into your bank, build a house
