Process for making double tier three

Just so I don't screw up and lose all my shards.
When making a double tier three item first I make both shards. How do I combine from here?

Do I put both in with my item and go or do I have to cimbine them together into a single shard first?

combine: essence 1, gem 1, focus 1, shard 1, high power cell

combine: essence 2, gem 2, focus 2, shard 2, high power cell (remember: gem and essence much match!!!!!)

combine: power shard 1, power shard 2, high power cell

combine: mixed power shard, item, high powercell

I thought that was the method but wasn't 100%. I almost missed the matching essence part when first reading about crafting but luckily I caught it before I started planning out my item.
