If I am too far away from an AOE spell (firewall, acid fog, etc...), if a critter is in the process of taking damage from it when it times out, my client will crash.
I've seen this happen in a variety of situations.
1. Ghosts of Perdition - I'll use a Dimension Door to escape back to the entrance if Cholthulzz is beating on me too hard. Since I normally have AOE in place to do damage, this is a guaranteed crash.
2. Inferno of the Damned - I made the mistake of using a firewall on a portal. Apparently, the portal on the other side respawned before the fire wall expired.
3. Reaver's Fate - I left an Acid Fog and Wall of Fire on the Reaver as I flew to throw the switch. Barely finished pulling the lever before my client crashed, and couldn't get back in before my fly expired.
Of course, the solution is to stay near your spells until they expire.
