I'd recommend perhaps changing your build design a little to something like this:
http://forums.ddo.com/showthread.php?t=156605
It'll do most of what I assume you are after in the build (high AC, healing capacity, melee capability, good saves etc), but it will also be a little more future-proof, in that it will get level 9 spells (in particular Mass Heal) when the level cap goes up to 20.
If you are set on the Clr12Pal2Mnk2 however, I'll have a go at it, based on your answers to the following questions:
How much do you see yourself meleeing?
a) Seldom - if I have a weapon out, something's gone wrong
b) Quite a bit, trash mobs only
c) All the time, trash mobs and bosses on normal and hard
d) All the time, trash mobs, bosses (all difficulties) and raid bosses on normal
e) All the time, including raid bosses on Hard
f) All the time, including raid bosses on Elite
Would you prefer to be...
a) Hard to hit with modest HP or
b) Not as hard to hit, but with stellar HP
Do you aim to get your saves...
a) High enough to be great at current endgame or
b) As high as possible, in case the future brings DCs in the 40s
Do you want to PVP at all?
a) Never
b) Seldom
c) Hell yeah! Make this toon around PVP.
How much loot do you have access to?
a) Just the basics - +6 stat items, +1 tomes, items of similar value
b) Good stuff - +6 stat rings, some but not heaps of +2 tomes, etc
c) Great stuff - Boots of the Innocent, Bloodstone, full set of +2 tomes, 24 large ingredients, etc
d) Uber stuff - you have a whole second account full of mules holding nothing other than W/P rapiers and stacks of 100 Large ingredients

Drop the second level of Paladin, you don't really need it and it prevents you from getting level 9 spells at endgame.
I have a build pushing 70 AC that's 13/2/1, here's a link: http://forums.ddo.com/showthread.php?t=154223

The bladestorms have no Save or evasion check. The only thing that gets you through them are Hit Points. Or avoiding them all together.
If bladestorms are your bane, you just need more hit points.

I like your build over iruka's although I would swap mental toughness for empower healing so you got some decent healbot capabilities. Than at level 18 you can either pick up maximize or heighten because your dcs are actually pretty good..

I may swap in Emp Healing once I hit the shroud - generally not worth it until then IMO. I honestly haven't thought about level 18, but it sounds like a good option.
