When crafting an item with 2 tier III shards, do you insert 1 at a time or both. I have enough ingredients for a tier III item but want to put a second tier III effect on it, can i craft the item and then put the second tier III on it later when i get enough ingredients?

For an item to get the Dual Shard Tier III effect, it has to be the same as the first two tiers. IE. :
Tier 1 - Fire Dominion Material
Tier 2 - Earth Dominion Material
Tier 3 - 1 - Fire Dominion Material
Tier 3 - 2 - Earth Dominion Material

Put the Tier 3 - 1 Shard and the Tier 3 - 2 Shard in the device with an energy cell and activate to combine them into one Uber-shard. Then, take that shard and your weapon and an energy cell to make the uber-weapon tier 3 dual shard effect.

In answer to your question : No. Because you have to combine the two shards to get the combo effect.

The exception to this is the route that combines all four elements.

No, you can't add the second tier 3 upgrade afterwards, it has to be done in one shot.

Note that the post above this one may be misleading, as only focuses have to match to get the special bonus, e.g. Fire / Earth / Fire + Earth, but you could have Fire / Etheral / Dominion on tier 1 and use Fire + Earth / Material / Opposition on tier 3.
However, it is widely assumed (although not 100% tested AFAIK) that to combine your two supreme shards, their essence and gem must match (in this example, combine Fire / Material / Opposition with Earth / Material / Opposion).
