AN INTRODUCTION
There have been several other threads started on this subject, and all are valuable... indeed, the work put forth by others in compiling information early on, as well as those who have contributed information on their experiments in those threads and elsewhere in the forums has made much of what is contained in this guide possible. They are the real pioneers and explorers in this endeavor, and should be accorded much gratitude for their efforts. I for one, thank all those who have made efforts in this regard.
I have found myself taking notes on their work, because of the way in which I process information. This guide, while containing observations of my own that I feel may help others in the process of crafting, varies not so much in content from the others as it does in it's style of organization. You may or may not find this style useful. It is useful to me, and I offer it here to others in the hopes that those who may still feel confused about the crafting process may find the answers they are seeking. If you have a question in regard to anything I've posted, or have unanswered questions after reading this guide, please feel free to ask and I will do my best to answer.

THE TWO TYPES OF ELDRITCH DEVICES/ALTARS
THE ALTARS OF CHANGE (A.K.A. "The Stones of Change")
Locations: Marketplace Bank (Right of Entrance), The Twelve Headquarters (Right of Entrance), Three Barrel Cove (Inside The Black Loch, Near the AH and Bank)
These altars allow you to enhance existing items. These altars vary from the other type in that 1) while you may be able to upgrade an item several times at an altar, you may only take one line of enhancement for each item, and 2) these altars can be used on both Green Steel and non Green Steel items. There are several of these altars, and they all have the same function, so it does not matter which one you use to enhance your items.
THE GREEN STEEL ALTARS
Locations: The Vale of Twilight Exploration Area: The Town of Meridia, The Shroud Raid
These altars allow you to create and upgrade Green Steel items. Each of the four altars has a different function. The first allows you to create a Green Steel weapon or accessory and is located in Meridia, the other three are located within the raid itself and allow you to upgrade the item you created in Meridia.
This guide presents each of the "Green Steel Altars" in order, each meriting it's own post. The posts for each altar are comprised of two main sections:
"SUPPLIES": A list of ingredients that apply to that altar, their location, and the keys to combining them.
"THE CREATION/UPGRADE TREE": This talks about what is imparted to your weapon or accessory by each ingredient. While encompassing, it is geared to be simple to use and understand.Following these, there is an appendix listing the focus combinations, and their resulting bonuses.
OTHER IMPORTANT INFO RELATING TO GREEN STEEL
Bound?
Once created, Green Steel weapons and accessories can be traded, sold, or auctioned, as long as you don't equip them. Once equipped, the item is permanently bound to you. You cannot trade, sell, or auction Green Steel weapons or accessories once they have been upgraded at any of the altars found in the raid. All other ingredients relating to Green Steel manufacture or upgrade, with the exception of the Essence of Cleansing, Shavarath Signet Stone, Shavarath Stones, and any of the Power Shards, are unbound, and may be traded, sold, or auctioned. The Essence of Cleansing, Shavarath Signet Stone, Shavarath Stones, and Power Shards are bind on aquire.
Removing the Taint of Shavarath
Green Steel has a thirst for pain and suffering. Wearing more than one Green Steel accessory at a time hurts the wearer by draining hp over time. To remove the taint, combine your item with an Essence of Cleansing at the final altar within the raid. The Essence of Cleansing can be found as an end reward on your 20th, 40th, 60th etc. Shroud completion, and can be used at any point in your crafting process. Green Steel Weapons do not affect you in this way and do not need to be cleansed.
Stacking
This does not cover the unique three and four focus enchantments because most of the information we have on these is based on user testing rather than being in the description of the item. Information from users is very limited and often has not included stacking information, or very limited stacking information at best, which may or may not be accurate.
Elemental Energy (HP): Stacks with everything but itself.
Improved Elemental Energy (HP): Stacks with everything but itself.
Greater Elemental Energy (HP): Stacks with everything but itself.
Weapon Damage Typing: These do stack with one another. However, resistance to and/or absorption of the energy/element will be applied to each instance of damage rather than the cummulative total. Note that equipping a weapon with Unholy/Evil damage typing will result in a negative level for anyone who is not of Evil alignment. The negative level remains in effect for as long as the item is equipped.
Minor Guard/Lesser Guard/Guard: Do not stack if same type. Strongest of a single type wins out. (This does not cover the unique guards that result from the 3 and 4 focus enchantments.)
Wizardry VI (SP): Does not stack with other Wizardry/Power items. Highest SP bonus wins out.
Elemental Spell Power (SP): Stacks with everything but itself.
Greater Elemental Spell Power (SP): Stacks with everything but itself.
Improved/Greater/Superior VI: Do not stack with each other on the same line of spells. Highest % on a single line of spells wins out.
Lore/Greater Lore/Superior Lore: Do not stack with each other on the same line of spells. Highest % on a single line of spells wins out.
Lesser/Improved/Greater Regeneration (HP): Do not stack with each other. Highest % wins out at any given time.
Incoming Healing Amplification:
Greater Resistance: Does not stack with itself when applied to the same element. Differing elements will work.
Inherent Resistance 5: Stacks with everything but itself, unless you vary the element.
Inherent Resistance 10: Stacks with everything but itself, unless you vary the element.
Energy Absorption:
Deflection Bonuses (AC): Do not stack with each other. Largest AC bonus wins out.
Insight Bonuses (AC): Do not stack with each other. Largest AC bonus wins out.
Competence Bonuses (Skills): Do not stack with each other when applied to the same skill. Highest bonus to a single skill wins out.
Enhancement Bonuses (Stats): Do not stack with each other when applied to the same stat. Highest bonus to a single stat wins out.
Resistance Bonuses (Saves): Do not stack with each other. Highest bonus to a single save type wins out. Example: If a +5 overall resistance and a +7 Reflex resistance is present, Fortitude will receive +5, Reflex +7, Will +5.
Exceptional Bonuses: Stacks with everything but itself. It will stack if a differing stat/save is used. Example: Charisma Exceptional Bonus +1 (Stat) will stack with Charisma Exceptional Bonus +2 (Stat), but not another Charisma Exceptional Bonus +1 (Stat). Charisma Exceptional Bonus +1 (Stat), will stack with Strength Exceptional Bonus +1 (Stat). Similarly, Charisma Exceptional Bonus +1 (Stat Based Skills) will stack with Charisma Exceptional Bonus +2 (Stat Based Skills), but not another Charisma Exceptional Bonus +1 (Stat Based Skills). Charisma Exceptional Bonus +1 (Stat Based Skills), will stack with Strength Exceptional Bonus +1 (Stat Based Skills). Ditto for Reflex Exceptional Resistance +1 (Save). It will stack with Reflex Exceptional Resistance +2 (Save), but not another Reflex Exceptional Resistance +1 (Save). Reflex Exceptional Resistance +1 (Save), will stack with Fortitude Exceptional Resistance +1 (Save). Exceptional Bonus (Stat), Exceptional Bonus (Stat Based Skills), and Exceptional Bonus (Save) are not considered the same bonus, but have been grouped together here for the purpose of comparing how similarly these types of bonuses work in order to minimize confusion.Generalities of Ingredients
"Ore of Travail" tends to create weapons.
"Filament of Toil" tends to create accessories.
"Material Essence" tends to create effects useful for non casters.
"Ethereal Essence" tends create effects useful for casters.
"Gem of Escalation" tends to improve our characters in some way: stats, skills, hp, sp... ethereal affects sp, material hp.
"Gem of Dominion" tends to cover offensive power: ethereal essence affects spell casting, material affects physical damage from weapons/armor.
"Gem of Opposition" tends to cover defensive abilities: ethereal essence in general providing what casters are most weak to, while material essence in general providing what non casters are most weak to.Also note that raw and manufactured ingredients for each of the upgrade altars (beginning with "The Altar of Invasion") use the same key. It is only the level of the ingredients that vary from altar to altar, not how they are combined.
Example:
Inferior Focus of Air : A, B, S, T
Focus of Air: Air: A, B, S, T
Superior Focus of Air: Air: A, B, S, T

5 Shavarath Stones
4 Eberron Energy Cells, or 3 Eberron Energy Cells and either a Medium or a Large Splintered Horn
12 Raw IngredientsPower Sources for "The Altar of Fecundity"
Eberron Energy Cell: These can be purchased from the NPC at the altar. With the exception of the creation of the Shavarath Signet Stone, you must use either one of these, a medium splintered horn, or a large splintered horn, every time you use the altar.
Medium Splintered Horn: These may be used in place of an Eberron Energy Cell when you are combining manufactured ingredients in order to create specific Green Steel items. They can be found in chests in parts 2-3 of "The Shroud" raid. Please see the section entitled "The Creation Tree" for more information.
Large Splintered Horn: These may be used in place of an Eberron Energy Cell when you are combining manufactured ingredients in order to create specific Green Steel items. They can be found in chests in parts 4-5 of "The Shroud" raid. Please see the section entitled "The Creation Tree" for more information.
Shavarath Signet Stone: This is created by combining the five Shavarath Stones in the altar. Each of the five Shavarath Stones can be found in one of the end chests for the five quests in the Vale of Twilight. (Battle: "Running with the Devils", Foresight: "Rainbow in the Dark", Might: "Ritual Sacrifice", Strategy: "Let Sleeping Dust Lie", Victory: "The Coalescence Chamber") Once created, you can then use the Shavarath Signet Stone; 1 of either an Eberron Energy Cell, Medium Splintered Horn, or Large Splintered Horn; 1 Taper; 1 of either the Filament of Toil or the Ore of Travail; and 1 of either the Wonderous Balm, Jewel, Oil, or Scarab in order to create your green steel item.Raw Ingredients

Corrections/Additions to "The Altar of Fecundity"
If you have information I don't have or a correction to submit for this section of the guide, I will need a screenshot of the item, and a list of what was used to create it. Where text is BLUE, I have insufficient information, conflicting information, or information that I have not been able to verify with a screenshot. Where text is RED, this information is currently correct, but the developers have indicated that there is going to be a change, or the item is not working as intended. If I have the information on what the change is, when it will take effect, or when the item will be fixed, this will also be in red.

Shavarath High Energy Cell: These drop from chests in the raid. With the exception of charging Depleted Shavarath High Energy Cells, one must be used every time you use this altar.
Shard of Supreme Power: These drop from chests in the raid. To imbue your shard, you must use, in any order: 1 Shavarath High Energy Cell, 1 Shard of Supreme Power, 1 Superior Focus, 1 Pure Essence, and 1 Flawless Gem. Once imbued, you can then use the Shard of Supreme Power, along with your item and a Shavarath High Energy Cell, to upgrade your item and have a chance at a 3 focus enchantment. Alternately, at this altar only, you can opt instead to create a dual imbued Shard of Supreme Power. This is achieved by imbuing two Shards of Supreme Power using the same method you have used thus far to imbue a shard. You must use identical essences and gems, which should match what you are trying to get from this altar, but you may vary the focus on each shard. To combine them into a dual shard, after you have imbued each of them, place them both into the altar with a Shavarath High Energy Cell. The resulting dual shard may then be combined with your item using another Shavarath High Energy Cell. This upgrades your item to include the effect(s) you've selected here, and gives your item a chance at a 4 focus enchantment instead of a chance at a 3 focus enchantment. Corrections/Additions to "The Altar of Devastation"
If you have information I do not have for this altar, or a correction to submit for this altar, I will need:
A Screenshot of the Item: The screenshot must clearly show that the item has obtained the third upgrade.
Essence and Gem Utilized: The other ingredients are not necessary because I can discern this info from the screenshot.Where text is BLUE, I have insufficient information, conflicting information, or information that I have not been able to verify with a screenshot. Where text is RED, this information is currently correct, but the developers have indicated that there is going to be a change, or the item is not working as intended. If I have the information on what the change is, when it will take effect, or when the item will be fixed, this will also be in red.

Please note that you may either receive a 3 focus enchantment or a 4 focus enchantment, but not both on a single item. This is because you can only upgrade your item once on the Altar of Devastation using either 1) a single imbued shard which may result in a 3 focus enchantment or 2) two imbued shards which have been combined into a single shard which may result in a 4 focus enchantment. I emphasize the word "may" because not all combinations will result in a 3 or 4 focus enchantment. The 3 focus enchantments that are known to work are:

Corrections/Additions
If you have information I don't have or a correction to submit for this section of the guide, I will need a screenshot of the item, and a list of ingredients used, in order of usage.

Example 1
Suppose you are a Cleric using Black Dragonplate Armor. This already imparts Wizardry V. Now lets say you have a set of Green Steel Goggles. You could go ethereal/escalation and either air or positive focus for Charisma based skills (lets say positive for this example). You would get from this Wizardry VI, +1 to your Cha based skills, and in this case, a positively aligned accessory at this point. Now, take into consideration that your wizardry items don't stack, and you are only getting +25 SP and +1 to your Cha based skills from this upgrade if you take your armor into account. Thats not really anything all that outstanding at this point.
Let's opt instead to go with something else, but lets find something that follows our intial choice to use positive focus. Hmmm... ok we could go with +10 hp and +1 to Con based skills, or we could choose lesser regen, or we could choose Lore for positive while still keeping our positive focus. Are any of these valuable to me and not something I already have on other gear I'm not likely to get rid of any time soon? For arguments sake, lets pick the +10 to hp and +1 to Con based skills, which gives us +1 to our Concentration.
Ok now we go with what we were originally thinking for the other two altars... ethereal/escalation/positive. Now at the next altar we get Elemental Spell Power +50 and +2 to Cha based skills, and at the altar following that we get Elemental Spell Power +100, which happens to stack with anything but itself...so the Elemental Spell Power +50 stacks with it, and +3 to Cha based skills, which again stacks with anything but itself. This gives us a total of +150 more spell points (in the case of a Cleric), and +5 to Cha based skills. We've also gone with three positive focuses. In the case of accessories, that gives us the following bonuses of a true rez clicky (life + 100% health) and disruption guard (when you are hit, chance to disrupt undead).
So we still get all the bonuses of an item that is a triple positively aligned item, we get all the sp (-25 for the difference between wizardry V and VI), and one less point to our Cha based skills. We however gain 10 more hp and +1 to our Con based skills, including Concentration. Or we could have gone for the lesser regen, which while slow, is useful for Clerics when in a unfamiliar party and no one has a way to bring us out of incap situations. (Just think of the savings in relics to your dragon armor with the new death penalty.)
As you can see from this example, you don't have to have the first line of this upgrade to get the benefit of that line in the second and third upgrades. This is especially important in the case of effects that do not stack. So you want to look at how the upgrades at each altar work with the gear you intend on keeping (Note that currently there is no option for Green Steel Armor, Shields, Rings or Trinkets at this time), as well as what bonuses you are shooting for.

really excellent guide! I enjoyed the step by step instructions with recipes.
I almost crafted both Altars of Invasion & Subjugation components without any error, thanks to your directions.
Unfortunately, I accidentally used the ethereal recipe at the Invasion Altar rather than the material recipe that I wanted (expected). Totally my fault as I read from the wrong recipe while I was writing down my own formula.
I wonder if you might use different color schemes in with your guide (rather than all white text)? Differentiate between Ethereal & Material, and any time Ethereal or Material is referenced you would use the corresponding color.
That might have saved my item. Or maybe not. :) Thanks!

Thank you. Although, it's still a work in progress, and much of the credit for it's content belongs to the population of DDO as a whole, not myself.
My purpose in keeping most of the text white was out of consideration for those with vision problems who need the highest contrast possible to read. That's also a part of why I used quotes... to darken the background. Yours is a good suggestion though and I can see cause for it. If I can get some input from those with vision problems as to what colors may work for them without hurting their eyes or degrading their ability to read the information too much, I'll do so. Thanks = )

Material/Dominon/Fire - Tier 2
Material/Dominion/Positive & Fire - Tier 3

We can see that Fire has the Dominating Essence for Radiance II
Would like to thank all my Exodus guildies for their support as well as a few others: BIG thanks to Kleio, Woodsmen, Urutz, my nizzle Owenn, Horseshack, BloodyRag and Bornwinter & Ilandrya for their patience in helping me craft, last but definately not least SUPER thanks to Candela for helping me run all those fun Shroud runs!!!! BTW: this weapon is rediculous and can confirm that rogues sneak attack was getting full benefit from the blinded enemy ;)

I'm guessing everyone and their mom will want a radiance 2 weapon.
Please tell me if this math is right for the 2 Supreme Shards:
(Large Ingredients):
4 Arrowheads
4 Bones
6 Scales
3 Stones
5 Shraps
2 Chains
--------
24
If you don't re-roll a 10 Rogue / 6 Ranger (Tempest) halfing with as many Sneak Attack enhancements as possible you're nuts :)

IF you want the flaming blast effect on a weapon along with radiance II, those would be the correct ingredients to create the appropriate dual shard. If you want something other than flaming blast along with radiance II, no.
Also, you will need to have used a positive focus and a fire focus on your first two upgrades (focus 1 and focus 2) at the earlier altars (resulting in radiance I) in order to be able to get radiance II. It doesn't matter which order they are in as long as you have used both focuses.
If you have any other questions or are confused at all, please ask... I want to make sure people are clear on things prior to crafting so they don't lose ingredients, and I don't want to assume anyone's level of crafting experience.

There is no result listed for the 4 focus combination negative aspect/negative aspect. Does that mean it doesn't do anything or that noone has tried it yet?

No one has tried it yet. The successful 4 focus enchantments have screenshot links next to them.
Ones suspected to POSSIBLY work that have not been tested are listed, but do not have screenshot links next to them. Do not use those unless you are willing to experiment and potentially fail/lose ingredients.
Ones that have failed are not listed. Also, if there is no reason to suspect a combination may work, it is not listed. This is due to the large number of possible combinations, the fact most people don't want to sacrifice a large number of large ingredients to test every possible combination, and the time it would take to collect enough ingredients to do so. As we come to suspect other possible combinations as plausable for testing, they will be listed, but only the ones with screen shot links are confirmed to work.

Got it. Thanks for the reply and the very useful post!
I'll keep checking back, b/c I'm sure someone else will have tried it by the time I get all the ingredients anway.

anyone looking at getting into green steel crafting may also want to consider http://www.iro.umontreal.ca/~delallea/ddo/shroud/index.php. that site has a very comprehensive list of available options, and in terms of creating a three tier green steel item, will ask what you want your weapon to do, and calculate exactly what you need to make it happen, then tell you exactly what ingredients you should collect, and how you should combine them to get what you want out of your weapon/accessory
